Colonial Oaks Skilled Nursing & Rehabilitation outpatient therapy offers senior adults continuing rehabilitation from injury, illness, or surgery after returning home. It is our goal to create a care plan that allows each patient to experience a smooth transition from inpatient to outpatient rehab.
